Determine if your position is eligible to work remotely...
- Talk to your supervisor about your Self-Assessment and requests.
- Complete the Employee Remote-Ability Self-Assessment to determine if working remotely is desirable and sustainable for you.
- Remember, your job description, not your preference, is ultimately what determines if working remotely is appropriate for your department.
- Complete a home office evaluation for ergonomics and equipment. This includes taking the "Ergonomics Self-Evaluation for Remote Work" LMS course. (There are also several other ergonomic LMS courses you can take.)
- If you are requesting any medical accommodations as a result of your own health condition, speak with your supervisor and consult with your OCP HR Analyst, who will coordinate with Disability Management Services.
- After you have had a conversation with your supervisor, and if you have come to a mutual agreement, fill out the necessary Flexible Work Arrangement forms:
- We suggest you use the "*NEW* Combined Flexible Work Schedule and Agreement" version, found here.
- Approver 1 is your unit's MAG member and Approver 2 is Katya Rodriguez [email protected].
- Or, if you are part of a large OCP unit with multiple levels of approvers, you may use the OCP version, which allows for an additional level of approval.
- Approver 1 is your unit's MAG member. Approver 2 (optional) is your Department Head. Approver 3 is OCP HR, Katya Rodriguez [email protected].
- We suggest you use the "*NEW* Combined Flexible Work Schedule and Agreement" version, found here.
- Complete the Employee Agreement Concerning Use of University Resources form for any University-issued office equipment that is taken off-site. Send the completed and signed form to [email protected].
Supervisor Guidance
As your department considers its business needs and in-person office plans...
- Review the Flexible Work Planning Guide.
- Determine if positions—not the employee—are suitable for flexible work by completing the Remote-Ability Position Assessment.
- Evaluate any Employee Remote-Ability Self-Assessments your employees may submit to you.
- Don't make any promises to your employee. Consult with your OCP HR Analyst, as there may be important ramifications to consider.
- If mutually agreed, have your employee fill out the Flexible Work Arrangement form.
- We suggest they use the "*NEW* Combined Flexible Work Schedule and Agreement" version, found here.

- After you’ve reviewed the flex work agreement, please review that the work space assignment is compliant with our campus guidelines. Make adjustments accordingly.
- If you have a cubicle or office to spare (permanently or temporarily), please let us know. Email OCP Business Operations.
- If you are making space changes, let us know so we can help. Email OCP Business Operations.
- Review and sign your employee's Employee Agreement Concerning Use of University Resources form. Have the employee send this to [email protected].
