1. Selected computer equipment that you want from http://itps.ucdavis.edu/OCP/standardsystems.php
2. Billing code
3. Three one hour time slots of availability to install the computer (there's a good chance it's in stock now. If it's not in stock they will order it and schedule another time to drop off the computer)
4. The location where you would like the computer equipment to be delievered
5. A contact name, email, and phone number - I would like to Onboard a new employee
